Getting views from members about our local society

Our local society is now 40 years old and we are conducting a review to see if our remits, policies, objectives etc are still valid or need some tweaking. We also want to know what our members expect of us e.g. websites, email lists, regular talks and social events, etc. We want to do this via a hard copy questionnaire to all our members (about 1000+). We are especially concerned that our age profile is getting older by the day and we do not seem to have much to offer to younger prospective members. Has anybody done this successfully and do they have a sample questionnaire that we could build on?

We will be talking about how local groups can best attract new members and volunteers at our AGM in Sheffield in October.  This will include a presentation on new research which has worked with groups in Leeds and Leicester and workshop discussions with other civic societies.  The emerging results are fascinating.
